FREQUENTLY ASKED QUESTIONS:
1) What are the directions to your office at 400 Executive Center Drive, West Palm Beach?
From the north:
Take I 95 South to the “Palm Beach Lakes Blvd.” exit. Go east (left turn) and make a right turn at the first light (BP Gas Station) at Executive Center Drive. (This is the road to Best Buy and Brand’s Mart). Go South on Executive Drive for 1 mile, go past the Congress Ave. light until you reach 400 Executive Center Plaza which is on your left hand side of the road. Take the elevator to the 2nd floor and make a left to Suite 202. The office is located on the second floor of the suite at the top of the stairs.
If you want to take the streets, take Congress Ave. south until you reach Executive Center Drive. Make a left turn and proceed to 400 Executive Center Plaza.
From the south:
Take I 95 North to “Okeechobee Blvd.” exit. Go west until you reach Congress Ave. Make a right (north) turn, go over I 95 until you reach the light at Executive Center Dr. Make a right on to Executive Center Dr. for approximately 2 blocks. The building is on your left hand side. Take the elevator to the 2nd floor and make a left to Suite 202. The office is located on the second floor of the suite at the top of the stairs.
2) Should I arrive early for my first visit to complete an intake form?
Yes, please arrive at least 15 minutes early to complete a medical intake form.
3) Do you have gift certificates available?
Yes. I would be happy to mail any gift certificate to your special loved one.
4) Do you take credit cards?
Yes. I take Visa and MasterCard as a payment option.
4) What is your cancellation policy?
Cancellation Policy: Please call to cancel within 4 hours or a 50% fee will be assessed.
